Please review the State of Illinois requirements for the specified grade levels and transfer students. All required exams and immunizations are to be returned to the school nurse. Failure to comply with the Illinois School Code requirements may result in your child's exclusion from school. All forms may be found on the school website www.dist228.org under parent/students. Please call one of the Geneseo school nurses if you have questions.
Kindergarten
-School physical with the student's up to date immunizations. Medical health history completed and signed by a parent and the physical exam signed and dated by the physician.
-Lead Screening is required for your child one time prior to entering kindergarten. The doctor or nurse must sign the Lead Risk Assessment questionnaire
-Dental Exam
-Vision Exam
2nd Grade
-Dental Exam
5th Grade
-Hepatitis B Series- If your child has not already had the Hepatitis B series it will be required at this grade level.
6th Grade
-School physical with the student's up to date immunizations. Medical health history completed and
signed by a parent and the physical exam signed and dated by the physician.
-Dental Exam
9th Grade
-School physical with the student's up to date immunizations. Medical health history completed and signed by a parent. Physical exam signed and dated by the physician. This physical will also serve as your child's sports physical for the upcoming year. IMPORTANT PLEASE NOTE: If your child is in a spring sport the physical should be dated after June 1st in order for it to be acceptable for next years entire spring season.
-Tetanus-Diphtheria (Td) booster vaccine: The State of Illinois requires a Td booster every ten years. Since most of the students entering ninth grade had their last Td with their kindergarten requirements it is now time for their Td booster.
Transfer students from Out of State
-School Physical- Required regardless of grade level when entering for the first time to an Illinois school
-Vision exam For all out of state transfer students entering an Illinois school for the first time
2012-2013 NEW IMMUNIZATION REQUIREMENT
ATTENTION PARENTS OF SIXTH AND NINTH GRADE
The Illinois State Board of Education in conjunction with the Illinois Department of Public Health has instituted a new health requirement rule. All students entering sixth and ninth grades must provide proof of having received the Tdap vaccination along with the required school physical.
Students in 6th and 9th grade without one of the following will be subject to exclusion:
1. Proof of Tdap vaccination
2. An approved medical or religious exemption on file with the school,
