Use of School Facilities

Please fill out all information completely.  Upon approval of the dates and times by the building administrator, this form will be forwarded to the Director of Operations at the District Office and a contract will be drafted for your signature prior to the event.  One-month notification is required for rental of any district facility.

It is understood that charges may result for utilities, custodians, and/or building rental.  Please see District Board Policy 8:20, 8:20-AP, and 8:20-AP2.  The renter shall provide to the District Unit Office a certificate of insurance of public liability, naming the district as certificate holder, in the amount of $1,000,000/$1,000,000 and property damage in the amount of $500,000.  Even if you have scheduled a facility/building in advance, a school district event/activity may take precedence over a non-district event. 


A $100.00 deposit is required and a current Certificate of Liability Insurance will be due to the District Office upon approval of this request.
