Physicians:---Hammond Henry Hospital Medical Staff             
Please arrive 10 minutes prior to your time to start preliminary screenings before the physicians’ exam. These are approximate times to stagger start the physicals.

You may pick up the new IHSA physical form from the high school nurse’s office, coaches, high school main office or online at www.ihsa.org/forms or http://www.dist228.org/parent_student/health/PhysicalCard.pdf.   The forms are to be returned to your school nurse by 5/28/2013 with the history completed and signed by both parent and student.  Both the parent and the student signatures are mandated by IHSA.  Please sign both the front signature line and the back signature line for both student and parent.  Students must have the proper signatures before being seen by the physician.
